B.2 STATEMENT OF WORK Background The Edward Hines Jr. VAH in Hines, Illinois, has a need to rent/lease a Modular Trailer for the Emergency Department / Emergency Room (ED / ER) for customers that include, but are not limited to, patients, visitors, clinic staff, and administrative employees. ANSWERS TO VENDOR QUESTIONS: 1. Is new construction required or is like-new acceptable? Newly fabricated not required, but detailed description and proof of condition for like-new shall be required. 2. What is the requested POP for the project as that information appears to be blank on the bid documents? One year base with 3 one-year options. 3. Section B.2 Statement of Work, under requirement states the contractor is to provide compliant decks, steps and ramps. Can you please clarify what type of material is to be used whether it be aluminum or PT wood? NOT PT wood. Aluminum needed. Needs to be slip resistant, diamond plated. 4. Section B.2 Statement of Work, under requirement states vinyl tile for the flooring but there is no mention of the other finishes. Industry standard finishes are VCG for the walls, FRP at wet areas, and 2x4 ceiling grid for ceiling. Are these acceptable? Yes, These are acceptable. Surfaces need to be cleanable in the event of spills. 5. What exterior siding is being requested? Smart-panel, hardi-panel, 26 GA hi-ribbed steel? 26 GA hi-ribbed steel is ok. 6. Section B.2 Statement of Work, under requirement states the contractor would be responsible for utility connections. Is there a utility survey which shows the tie in locations for the water, sewer, electrical, comm, etc.? We have a campus-wide utility survey that can be supplied upon request. 7. Section B.2 Statement of Work, under requirement states a fire alarm is required. Is the intention of the Fire Alarm system in the modular building to match the Fire Alarm system on the existing building 111? If so, what is the current/ existing system? We have a JCI Simplex Reportable fire alarm system in the building. We can use a stand-alone system if needed. 8. We would intend to propose the low voltage wiring for the Fire Alarm System to be ran above ceiling with plenum rated cable. Is this acceptable in the modular building or is hard conduit required? Hard conduit not required. Propose this be ran above ceiling with plenum rated cable. 9. What is the required data/comm configuration per drop for the modular building? Is there current use of a VOIP system? Our standard configurations is 2 cables per drop. (1) data and (1) voice, per drop. Is this acceptable? 2 data drops and 2 phone drops is preferred. 10. Section B.2 Statement of Work, under requirement states a covered walkway is required. Does this pertain to the requested decks, steps and ramps or is this separate? If separate, is there a site map which shows the distance of this covered walkway and also are sidewalks required as well? Whole area is paved. Yes, this pertains to decks, steps, and ramps. Covered deck, steps, and ramps. Vendor needs to provide site map to us since they are placing this on site. 11. It is our understanding that all water piping outside of building envelope will need to be protected from freezing. The exposed areas on the modular building would only be in the crawlspace below the sub floor. Are both pipe insulation and heat trace required, or just insulation? Also, it is our understanding that this is only required on water supply lines and not sanitary sewer lines. Please confirm. There is no water piping. 12. Is the existing power on-site three phase or single phase? Both are available. 13. Is there an existing main distribution panel on site for us to connect the subpanels to? If so, what is amperage of this existing panel? Yes, there is an existing main distribution panel. Normal 30-amp feeds. 14. Will emergency power be required for the modular building? No. 15. Are there any other special low voltage systems we need to be aware of? i.e.. Sound Mask, VSS, CCTV, etc.? No. 16. Is there a geo-tech report or topographic survey that is available? This is available on google-earth. 17. Can you identify the room requirements for the temporary modular facility? How many exam rooms, restrooms, offices, etc.? Will an X-Ray room be required? 8 rooms total is ideal. If needed, one of the patient rooms can become an office.
