Description
**THIS IS A BUY INDIAN SET ASIDE per Buy Indian Act, 25 U.S.C. 47 and only offers form Buy Indian small businesses will be accepted.
Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 95-13 Personal services Contracts.
This solicitation notice is a request for proposals “Non-Personal Service” contract for two (2) MCH OB/GYN Peds Out Patient Registered Nurses to provide services at Northern Navajo Medical Center, (NNMC) U.S. Hwy 491 North, Shiprock, NM. Services shall be performed in accordance with the attached Performance Work Statement (PWS).
The Period of Performance will be issued for 6 months each will have 1) Base Period 2) with an Option 1 Period. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the nurse will be released from contract assignment.
The North American Industry Classification System or NAICS code is 561320 with a small business size standard of $30M.
This solicitation is using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures.
In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
EVALUATION CRITERIA: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant factors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. All candidates are required to have an official United States social security number. Do not send a copy of RN’s social security card with profiles.
SUBMITTAL REQUIREMENTS:
Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. In addition, any profiles received before or after the RFQ dates WILL NOT be accepted.
** It is the responsibility of the Offeror to submit only qualified, applicable, and available personnel. Any personnel submitted by multiple Offerors will not be forwarded for evaluation and further consideration. **
- IDENTIFY profile submission with solicitation Notice ID number only.
- Attached each RN complete profile: Curriculum vitae, resume, licensures, and all relative documents for proposed candidates. currently/valid contact information- phone number/email address. MCH OB/GYN certification required is BLS.
- Provide a complete and sign Pricing Schedule (attached). The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local).
- Provide evidence of Provider’s Experience of comparable scope and complexity in providing services within the past three (3) years.
- Provide evidence of the Contract Company’s Past Performance in providing steady supply of candidates to fill the vacancies.
- Provide a completed IHS IEE Representation Form (attached).
- Certificate of Liability Insurance (COI) for each candidate
The Contractor will start until the Purchase Order is signed by Contracting Officer
