Western North Carolina VA Healthcare System Charles George VA Medical Center Asheville, NC Statement of Work 1.0 Background The Charles George VA Medical Center (CGVAMC) has a need to procure to replace all the facility s Medical Gas/Vacuum System suction regulators. Currently CGVAMC has outdated suction regulators and per national standards are required to be standardized. These Two Hundred and Three (203) Sentinel Suction Regulator, Analog, Continuous/Intermittent, USA, Tubing Nipple, DISS connection items and these Fifty-Nine (59) Sentinel Suction Regulator, Analog, Continuous/Intermittent, USA, Tubing Nipple, Chemetron connection items are being requested to bring the facility up to date. The current regulators are both manufacturer supported and to be in compliance with VA mandates to have back-up/duplicate regulators. Typically, these are time sensitive and unpredictable in nature. Having appropriate regulators to provide proper suction at the bedside for patients is critical. These are basic suction regulators nearly universal in the hospital setting and are a glaring deficiency in our current portfolio. 2.0 Scope The CGVAMC will procure Two Hundred and Three (203) Sentinel Suction Regulator, Analog, Continuous/Intermittent, USA, Tubing Nipple, DISS connection items and these Fifty-Nine (59) Sentinel Suction Regulator, Analog, Continuous/Intermittent, USA, Tubing Nipple, Chemetron connection. 2.1 This contract is all-inclusive. Prospective offerors should read the requirements carefully to ensure a complete understanding of the contract requirements to ensure their proposal includes all expenses and costs associated with performance related to the contract. The Government will not reimburse offerors due to a failure on the offeror s part to consider all costs associated with performance on this requirement. 2.2 All tasks associated with this order shall be completed during the CGVAMC s normal business hours, Monday through Friday from 8:00am 4:30pm excluding Federal holidays unless otherwise coordinated with the Contracting Officer s Representative (COR) or Surgery Staff. The current Federal Holidays for calendar year 2023 and 2024 are as follows: Friday, November 10 * Veterans Day Thursday, November 23 Thanksgiving Day Monday, December 25 Christmas Day Monday, January 01 New Year s Day Monday, January 15 Birthday of Martin Luther King, Jr. Monday, February 19 * Washington s Birthday Monday, May 27 Memorial Day Wednesday, June 19 Juneteenth National Independence Day Thursday, July 04 Independence Day Monday, September 02 Labor Day Monday, October 14 Columbus Day Monday, November 11 Veterans Day Thursday, November 28 Thanksgiving Day Wednesday, December 25 Christmas Day *Any other day determined by the President of the United States as a Federal Holiday. 3.0 Management & Supervision of Contract Personnel The Contractor shall be responsible for supervising the daily services provided under this contract. The Contractor shall provide the name and contact information for a person designated to manage and supervise performance under this contract to the CO upon award of the contract. 3.1 Personnel Policy The Contractor shall be responsible for protecting the personnel furnishing products and/or services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: - Workers compensation - Professional liability insurance - Health examinations - Income tax withholding, and - Social security payments. 3.2 Smoking Policy Smoking is strictly prohibited while on the CGVAMC campus. Violations can result in a fine of $50.00 or more. 4.0 Specific Tasks The contractor shall deliver one (1) set of updated versions of Master Shoulder Instrument Set for Rotator Cuff repairs set intended for shoulder rotator cuff repairs and instrument set in accordance to the attached quote. 5.0 INVOICING / PAYMENT: The Contractor shall submit invoices in arrears, covering the goods and/or services performed under this contract.
