This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for Market Research. This is a Request for Information only. This is NOT a solicitation for proposals, proposal abstracts, or quotations.
The Department of Veterans Affairs Olin E. Teague Veterans Medical Center Surgical Service in Temple, Texas intends to award a brand name or equal contract award for the purchase of Full-Service Agreement Olympus ENT Orbeye. The Government is conducting a market survey to help determine the availability and technical capability of qualified service-disabled veteran-owned small businesses, veteran-owned small businesses, small businesses, HUBZone small businesses and/or other large businesses capable of serving the needs identified below. This notice of intent is for open market as well as Federal Supply Schedule items.
The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business, relative to NAICS 811210 with a size standard of $34M. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov or GSA E-Buy at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible.

Full-Service Agreement for ENT ORBEYE Equipment
1.SCOPE OF WORK:
The Department of Veterans Affairs Central Texas Veterans Health Care System is requiring Full-service coverage for repair of Olympus brand ENT ORBEYE Video Microscopes, Electronics I and Electronics II, regardless of degree, reason, or repetitiveness of damage. The period of service for this contract is Base Plus Four (4) Option Years. This is an effort to procure reliable, expedient service for repair of scopes and electronics through Surgical Service and Biomedical Service. This will expedite the process for repairing of scopes and electronics by selecting one vendor, the Original Equipment Manufacturer (OEM).
2. LOCATION OF WORK:
The contractor will provide Service to maintain the current Olympus Video Microscopes Model OME-L200, OME-V200, and Electronics MAJ-2301, MAJ2303, LMD-X550ST and HVO-4000ST located in the ENT clinic at the Central Texas Veterans Health Care System at Temple, Texas.
3. WORK TO BE PERFORMED:
The contractor will provide service to cover the repair, service, and restoration to factory specifications of Olympus ENT ORBEYEY Video microscopes, and electronics regardless of cause of damage. The items are all located at the Central Texas Veterans Healthcare System Temple Campus and are used by Surgical Services in the OR. The list is as follows:

SECURITY:
C&A requirements do not apply, and a Security Accreditation Package is not required. All services for this contract must meet manufacturer performance and technical specifications, Federal Regulations, and other specifications that may apply such as National Electric Code (NEC), Life Safety Code (LSC), Joint Commission (JC), and VA Regulations. Also, Contractor must provide the appropriate service documentation at no additional expense.
4. SPECIAL WORK REQUIREMENTS:
Hours: Services shall be performed according to specifications of Government point of contact and COR. The regular work hours four this facility is Monday through Friday 0800am to 4:30pm (CST).
Noncontract Charges: The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer.
Property Damage: The contractor shall take all necessary precautions to prevent damage to any government property and will notify the Contracting Officer Representative immediately if damages occur. The Contracting Officer will authorize the contractor to remedy the situation in one of the following ways: be assessed current replacement costs for damaged property, replace damaged property in a timely manner, or correct the damages with like materials at no additional cost.
Identification, Parking, Smoking, and VA Regulations:
The contractor will need to acquire a badge for each worker and ensure that the badge be worn in a visible location on the worker at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited inside any buildings at the VA. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States Dis trict Court.
Documentation: All service documentation must contain:
Date and time of the contractors' arrival on station
Type, model, and serial number(s) of all equipment on which service was performed
Total time of performance period, excluding travel time
Detailed narrative description of reason for service performed to include reported problem and cause of problem (When applicable)
Complete list of parts replaced (When applicable)
Date and time equipment were returned to serviceability.
Personnel Qualifications: Contractor personnel performing under this contract will be fully qualified and competent for the equipment to be performed
5. TASK AND DELIVERABLES
Description of Tasks and Associated Deliverables.
Task #1: This contract shall cover all repairs/maintenance/factory restoration that is needed for all Olympus endoscopes and endoscope accessory equipment, regardless of cause of malfunction.
Task #2: The OEM shall be responsible for providing crates for endoscopes, shipping, labeling, and providing updates to the Biomedical Engineering department and Medical/Surgical Services on the progress of the endoscope repair.
Task #3: The OEM shall provide loaner endoscopes of an equal quality when repairs on a scope or electronic is necessary.
6. REPORTING REQUIREMENTS:
Olympus shall be in constant contact with Central Texas while maintaining and repairing the scopes so that CTX can plan patient procedures accordingly.
CONTRACTOR RESPONSIBILITIES
The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of TEXAS. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with the regard to any claims, loss, damage, injury, and liability resulting there from.
PERIOD OF PERFORMANCE: The period of service for this contract is Base Plus Four (4) Option Years.
