This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. (ii) Request for Quotation (RFQ) 36C10B22R0022 (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06. (v) See Section B below for the Schedule of Deliverables. (vi) See Section B below for the Performance Work Statement. (vii) The period of performance shall consist of a base period from date of award until November 30, 2023, when the system must be fully functional and be able to support all the NTP-specific requirements for NextGen PACS. After the base period, there shall be a total of three (3) 12-month option periods which include ongoing support, sustainment, and system optimization of the NextGen PACS, if exercised. See Section E below for Basis for Award. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. See Section E below for the addendum. (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. See Section C below for the addendum. (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See Section C below for the addendum.
