Description
The U.S. Army Corps of Engineers (USACE), Baltimore District, will procure a forest herbicide treatment for the Raystown Lake Project detailed in the Performance Work Statement. This solicitation will result in one award based on lowest price technical accepted as best meets the needs of the Government. The proposed contract will run from date of award to May 31, 2023. Site visits are not available for award. The Government will not reimburse any cost incurred.

Quote Requirements
The quote shall be submitted with a cover page, first page of the SF 1449 solicitation, and block 30 completed, and three volumes in Microsoft Word Document or Adobe Acrobat Document format. The cover page shall include the quoter’s CAGE Code, TAX ID, and DUNS number. Each volume, as described below, shall be appropriately labeled to present a clear and organized quote:
VOLUME I: TECHNICAL CAPACITY: (NO PRICING INFORMATION SHALL BE IN THIS VOLUME)
Equipment plan: The proposed equipment list should only include the equipment which will be used on this contract. Include size, make, model, and ownership (owned or rented) of equipment. Include proposed herbicides to be used for treatments in this section.
Staffing plan: The proposed staffing plan shall include both numbers of personnel proposed and position titles.
Work plan: A work plan shall be submitted indicating how the work will be accomplished including the steps required and definable features of work. An anticipated schedule of work in compliance with the Performance of Work Statement should also be provided and shall include the earliest response time from date of award.
VOLUME II: PAST PERFORMANCE
Quality of recent project experience from the customer’s perspective. The Offeror shall arrange for the reference customer point of contact for three projects to fill out the questionnaire and return directly to Ashley Flaherty, Contract Specialist, via e-mail at ashley.m.flaherty@usace.army.mil. The questionnaires are forwarded directly by the references. The Government will not release the questionnaire forms to the Offeror at any time, in order for the Government to solicit candid, unbiased responses and comments. Two references from previously completed jobs are requested with a point of contact and phone numbers. These references will be relevant to the work which will be performed in this contract. References will be within the past 5 years. Note providing references is NOT required for this award and not providing references will not disquality a bid.
VOLUME III: PRICE
Provide Section B of the SF 1449 completed with prices. The Contractor shall provide all necessary equipment, staff, labor, supervision, materials, equipment, and supplies to complete the project.
