Description

The Indian Health Service, Gallup Indian Medical Center, located in Gallup, New Mexico 87301, has a requirement for Blanket Purchase Agreement(s) for various and miscellaneous medical supplies on an “As Needed Basis” for a base period of 12 months and an option to extend for an additional 12 months. The North American Industry Classification System or NAICS code is 339113 with a small business concern of $16.5. The agency will be placing multiple Blanket Purchase Agreements (BPAs) with the following vendors: Masimo Corporation, Medline Industries, Method 1 Solutions, Cardinal Health, Chyoce Products, Stryker Sage Products, Welch Allyn, Nihon Khoden, QB Medical, Fisher Scientific, KCI, Beckton Dickinson and Company, Johnson and Johnson, Bard Peripheral, BARD Medical, Mainline Medical, Vyaire Medical, Tri-Anim Health, and Mid Central Medical Corp.  Any other vendors interested in setting up BPAs for the aforementioned needs are encouraged to submit their capability statement, price lists and catalogs to Gallup Indian Medical Center – Acquisitions, email: Stefanie.atene@ihs.gov by January 31, 2022 at 10:00 am. If future needs arise, capable vendors may be contacted for additional BPAs. In accordance with: 52.204-7(b)(1) System for Award Management. An Offeror is required to be registered in SAM when submitting an offer or quotation, and shall continue to be registered until time of award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. 52.204-13(c) System for Award Management Maintenance The Contractor shall maintain registration in SAM during contract performance and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement. The Contractor is responsible for the currency, accuracy and completeness of the data within SAM, and for any liability resulting from the Government's reliance on inaccurate or incomplete data. To remain registered in SAM after the initial registration, the Contractor is required to review and update on an annual basis, from the date of initial registration or subsequent updates, its information in SAM to ensure it is current, accurate and complete. Updating information in SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document. 52.211-6(a) Brand Name or Equal If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government’s needs.
