OFFEROR READ THIS INFORMATION FIRST BEFORE SUBMITTING AN OFFER NOTICE TO OFFEROR: This is a difficult position to recruit. Due to the critical need for Diagnostic Radiological Cardiovascular Technician Service at the Minneapolis MN VA Hospital, offeror should seriously consider their capability of being able to provide the services before submitting an offer. Inability to provide the required service will be annotated through Contract Discrepancy Reports and identified in the Government Contractor Performance Assessment Reporting Service (CPARS) which may affect future contract opportunities. Please ensure adequate and qualified candidates are available before submitting an offer. This a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is a solicitation conducted using FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26323Q0252 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-08 dated 10/28/2022. This solicitation is set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB) entities. At the time of submission of offer and prior to award, the offeror shall be listed as verified in VIP database at https://www.vip.vetbiz.va.gov and verified and active as a Small Business in the System for Award Management (SAM) reflecting 621111 Offices of Physicians (except Mental Health Specialists) with a small business size standard of $14.0 Million. The PSC Code is Q522 Medical Radiology. The VA anticipates the award of one (1) Firm Fixed Price (FFP) contract for On-Site 1.0 FTEE (2080 Hours) Diagnostic Radiological Technologist Cardiovascular (DRT-CV) service to support the Minneapolis MN VA Health Care System located at One Veterans Way, Minneapolis MN 55417. See Performance Work Statement and Price/Cost Schedule for specific requirements and qualifications. The period of performance is from March 1, 2023, to February 29, 2024, and includes four (4) one year option periods to be exercised at the discretion of the government. Solicitation Questions.
