Service Contract for SANIPAK Sterilizer/Compactor 3. Background. To provide monthly preventive maintenance for the SANIPAK Sterilizer/Compactor and service in the event of a breakdown. The factory warranty lapses on 31 October 2023. The contract should pick up the service the following day. 4. Scope. Contractor shall provide all labor, material, transportation, equipment, lodging and supervision necessary to perform Preventive Maintenance Inspections and Intervening Repair Services for the VA owned SANIPAK unit and peripherals located at Louis Stokes VA Medical Center, 10701 East Blvd. Cleveland Ohio 44106. Services shall be in accordance with the terms, conditions, provisions and schedule of the contract. Services shall be for the period of 1 November 2023 through 31 October 2024, with four pre-priced one-year options for annual renewal. 5. Specific Tasks. Task 1: Perform monthly preventive maintenance Provide 12 scheduled preventative maintenance calls Task 2: Perform Unscheduled/ Intervening Maintenance Response by telephone to service calls shall be made within 4 hours On station response to be guaranteed within 8 hours of receipt of service call Includes (12) unscheduled maintenance calls with (1) technician spending up to 6. Performance Monitoring. The Government Point of Contact will monitor the performance on this contract to ensure that contract requirements are met. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There will be no government furnished equipment provided for the purposes of this contract. 8. Packaging, Packing and Shipping Instructions. Any parts shipped to the VA facility will be addressed to the contractor with attention line that states the Government Point of Contact s name and contact info.
