Home-Based Sleep Study Services for Dayton VAMC The purpose of this sources sought notice is to seek responses from all qualified, prospective offerors interested in the forthcoming solicitation for the subject requirement summarized as follows: Short Title: Home-Based Sleep Study Services for Dayton VAMC NAICS-621610: Home Health Care Services Place of Performance: Patients home of residence in support of Dayton VA Medical Center (VAMC) Anticipated Periods of Performance: Base Year from 2/01/2024 to 1/31/2025 Option Year One (OY1) from 2/01/2025 to 1/31/2026 Option Year Two (OY2) from 2/01/2026 to 1/31/2027 Option Year Three (OY3) from 2/01/2027 to 1/31/2028 Option Year Four (OY4) from 2/01/2028 to 1/31/2029 Attachments for Reference: A draft version of the Performance Work Statement (PWS) is attached below for offeror reference as appropriate. Attachments for Reference: Current DOL Wage Determination for Montgomery County, Ohio. Request all prospective offerors interested in this requirement email their 1) company name, 2) UEI number, 3) CAGE code, 4) business socio-economic size status, 5) signed and fully completed statement of FAR 52.219-14 Limitations on Subcontracting Compliance and 6) Capabilities Statement describing how specifically your company intends to meet all requirements specified in the PWS in Adobe PDF format to bradley.burhite@va.gov not later than Wednesday 12/02/24 at 1500-EST. In the capabilities statement, all prospective offerors shall provide documentation of FDA approval for anticipated home sleep test devices and evidence that home sleep test results can be securely transmitted to the VA IT system which must be FedRAMP approved IAW VA Handbook 6517 for all Cloud-Based services to be responsive.. In addition, please note that a signed and full completed copy of the FAR 52.219-14 (Limitations on Subcontracting) compliance letter is required to be considered responsive to this source sought notice. Please note that this requirement is subject to availability of funds in accordance with FAR 52.232-18 (Availability of Funds).
