(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. (ii) Solicitation number 36C26123Q0691 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) This acquisition is Total SDVOSB Set Aside. The NAICS code is 561421. (v) The contract line items are listed in the price schedule of the solicitation package. (vi) The contractor shall provide Telephone Operator Services for the VA Palo Alto, VA Central California, and VA San Francisco Health Care Systems in accordance with the Performance Work Statement that is included in the solicitation package. (vii) The contract will be for a base year plus two option years. The period of performance is listed in the Price Schedule. (viii) The provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. See the solicitation package for this provision which includes the quote submission instructions. (ix) The provision 52.212-2, Evaluation Commercial Items, applies to this acquisition. See the solicitation package for the evaluation factors that will be used to determine which vendor will be selected for the contract award.
