NOTICE OF INTENT TO SOLE SOURCE The Network Contracting Office (NCO) 2, Department of Veterans Affairs intends to award a firm fixed price sole-source contract with CIVCO Medical Instruments Co, Inc., a large business, for the purchase of Stereotactic Body Radio Therapy (SBRT) Board and accessories for the following medical center: Samuel S. Stratton VA Medical Center 113 Holland Avenue Albany, NY 12208 This sole source determination is in accordance with FAR 6.302-1, only one responsible source and no other supplies or services will satisfy agency requirement. CIVCO Medical Instruments Co, Inc. is the only known authorized supplier and servicing agent for CIVCO SBRT Body Pro-Lok ONE Board and accessories. The North American Industry Classification System (NAICS) code for this procurement is 339113 and the size standard is 750 employees. No solicitation package will be issued. This notice of intent is not a request for competitive quotes; however, interested parties may identify their interest and capability to respond to this requirement. Interested vendors that can meet this requirement, should identify their interest and capability, and respond to this notice by 4:00 PM EST on May 05, 2023. Interested vendors must send a capability statement, no more than one page in length; to include Company Name, UEI Number, Socioeconomic Category, brief description of your business entities capability to provide these items.
