Description

This synopsis/solicitation notice is a request for proposals for a Non-Personal Service Contract for a Contractor to provide practical nurse services for Northern Navajo Medical Center, Highway 491 North, Shiprock, New Mexico, 87420 - Navajo Area Indian Health Service (IHS) throughout the duration of the contract period of performance. Services shall be performed in accordance with the attached Performance Work Statement (PWS).

This Request for Quote (RFQ) is Unrestricted for a Non-Personal Service Contract per FAR 37.401(a) for RN services to be performed at the Northern Navajo Medical Center, Shiprock, New Mexico of the Indian Health Service (IHS). The tasks to be performed are cited in the attached Performance Work Statements

Ambulatory Care – 2 positions

MCH OB/GYN-Peds Out Patient – 2 positions

The Period of Performance will be issued as Base plus Option Periods:

Base Period: Date of Award through 8/31/2022

Option Period: 09/01/2022 through 02/28/2023

The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the nurse will be released from contract assignment.

The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The Supervisor at IHS shall determine specific tour of duty.

The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated

Education: Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant

Experience: The practical nurse needs to have at least 2 years of experience in professional nursing care principles, practice, and procedures. Must have knowledge of immunization, diabetes, a background in a clinic situation with a primary setting from newborn to elderly care

  1. Return attached fill in pricing with the profile . Quote(s) submitted will be negotiated. The rate shall be an all-inclusive rate of hourly, travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local).
  2. e-QIP: Required Background Investigation Information (MUST BE LEGIBLE & COMPLETED. DO NOT RE-FORMAT) PLEASE NOTE: UPON COMPLETION OF eQIP, THE SIGNATURE PAGES MUST BE SUBMITTED TO HUMAN RESOURCE DEPARTMENT 4. ON THE FIRST DAY OF ASSIGNMENT INCLUDING TWO FORMS OF IDENTIFICATION.
  3. Profile(s) must include valid phone number and email address ***ONLY SUBMIT INDIVIDUALS WHO ARE AVAILABLE DURING THE DELIVERY SCHEDULE *
  4. Three (3) current references with active telephone numbers. The references MUST be noted in a separate Word or PDF document and submitted as an attachment.
  5. Required to float to other units on an as needed basis.
  6. Contractor will not start until the Purchase Order is issued - signed by the Contracting Officer
  7. Certificate of Liability Insurance (COI) for each candidate also be considered, should one or more quotes represent the best value

Certifications and Licensure (MANDATORY):

A valid and unrestricted State Practice Nurse License (Certificate)

B Basic Life Support (BLS) Certification with American Heart Association

Housing: No government housing available. There are motels, restaurants, mall, etc. in Farmington, NM , 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM.

CONDITION OF EMPLOYMENT: Background Investigation: All qualified referred candidates will undergo Fingerprinting process. Fingerprint Card: FBI FD-258 (provided by the Federal Government): Fingerprinting MUST be cleared and approved prior to contact assignment. Contract assignment(s) are conditional on a cleared background check. OPM requires Electronic Questionnaire for Investigation Processing (e-QIP) for registered nursing services exceeding One Hundred Twenty (120) days. Electronic Questionnaire for Investigation Processing (e-QIP): Executive Orders 10450, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM) system.

 Type of Citizenship: U.S. Citizenship is required.
