Amendment 0002 is issued to: 

1. To extend the submission deadline date from 16 January 2023 at 2PM to 18 January 2023 at 2PM, in observance of the federal holiday.  Submission of proposals and PPQs have been extended to 18 January 2023 at 2:00PM. 

2. Provide an updated government responses to vendor questions. No further questions will be entertained.

All other terms and conditions remain the same and unchanged.

Amendment 0001 is issued to: 

1. Provide vendor questions and government responses; see attachment amendment 0001_W81K00-23-R-0005 - Questions_Responses.  No further questions will be entertained. 

2.  Provide revised Attachment 2 - Pricing Worksheet. 

3. Corrected 0001AA CLIN Quantity.

The Medical Readiness Contracting Office - West, (MRCO-W), Fort Sam Houston, TX is issuing a Request for Proposal (RFP) to support Brooke Army Medical Center, Joint Base San Antonio to acquire a personal services contract to provide twenty (20) Full Time (FTE) Nurse Case Managers (NCM) to provide patient centered, individual, and family level case management.

This requirement is being solicited as an 8(a) competitive procurement using Best Value – Trade Off Source Selection procedures. Therefore, only 8(a) certified Small Businesses will be considered. The Government will be awarding a Firm-Fixed Price (FFP) contract with base period of twelve (12) months, 01 April 2023 through 31 March 2024, plus four (4), 12-month option years. The Government reserves the right to exercise or not exercise the option periods as specified. The North American Industry Classification System (NAICS) code for this solicitation is 621399 – Office of All Other Miscellaneous Health Practitioners. The Small Business Size Standard for this NAICS is $9M.

Offerors shall submit proposals in electronic form via email no later than 2:00 P.M., Central Time (CT)((San Antonio, Texas)), 16 January 2023. The point of contact is Ms. Tammi Lambert, Contract Specialist, at tammi.r.lambert.civ@health.mil and Robin M. Castro, Contracting Officer, at robin.m.castro.civ@health.mil.

Offers shall be submitted via e-mail only. Proposals received after 2:00 P.M., CT on 16 January 2023 will be late and will not be considered for award. Failure to submit the required documentation as specified in the following paragraphs will make the Offeror non-compliant and ineligible for award. It is vital that Offerors follow instructions provided and adhere to page limits specified.

Questions shall be submitted via e-mail to Ms. Tammi Lambert (tammi.r.lambert.civ@health.mil) and Robin M. Castro (robin.m.castro.civ@health.mil) not later than 2:00 P.M., CT 28 December 2022. No questions will be entertained after this date. Do NOT submit multiple emails for questions.
