Description
PLEASE NOTE: VENDOR QUESTIONS HAVE NOW BEEN ANSWERED ON THE ATTACHED AMENDMENT [A001] TO THIS RFQ
This is a 100% Total Small Business combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6. The USPTO plans to award a commercial item contract utilizing simplified acquisition procedures to the small business offeror that provides the lowest price technically acceptable quote able fulfill this requirement.
This acquisition will be processed under FAR Part 13 Simplified Acquisition Procedures (SAP). The Solicitation number 1333BJ22Q00050011 is attached. The solicitation is issued as a Request for Quotes (RFQ).
A summary of the Scope of this requirement is as follows:
The scope of the work involves providing the replacement of 75 HeartSine Samaritan Pad-Pak batteries and medical oversight for all 75 HEARTISNE AEDs assigned to USPTO.
Deliverables
This is what is expected before and during the period of performance for this contract.
- 75 AED Heartsine Samaritan Pad-pak replacement batteries
- AED event review and documentation by Medical director (Medical Doctor), to include EMS registration, and providing USPTO with Policy and procedures manuals, AED maintenance and CPR Training information, if and when needed.
- Site analysis for AED placement recommendations when needed.
Period of Performance
The period of performance will consist of a twelve-month base year and two twelve-month option years. Option years are for medical oversight. The batteries have a 3-4 year warranty. This can be obtained with a Purchase Card, if below the limit.
Specific Requirements and evaluation procedures are listed in the attached solicitation.

TECHNICAL PROPOSAL
• Please submit a separate proposal for technical evaluation as an attachment in your quote package
This proposal should provide enough information to show that your proposal meets or exceeds the requirements stated in the SOW

Please note that boxes 30a, 30b, and 30c on page 1 of the SF-1449 RFQ document MUST be completed and signed and returned with your quote to be responsive.
