Description

The Indian Health Service, Gallup Service Unit, located in Gallup, New Mexico, 87301 has an upcoming requirement for one (2) Full-Time Non-Personal Healthcare Services for General Surgeon in the General Surgery Department.

The period of performance will be as follows:

Base Period: 07/15/2022 - 09/30/2022

1st Option Period: 10/01/2022 - 12/31/2022

2nd Option Period: 01/01/2023 - 03/31/2023

3rd Option Period: 03/01/2023 - 06/30/2023

The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $12.0 M. 

The Request for Quotations No. 75H71022Q00029 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 06/21/2022.  Evaluation factors may include:

Required capabilities and performance characteristics:

1.            Residency in General Surgery

2.            Contractor’s experience of twenty-four (24) months. Endoscopy experience (upper and lower).

3.            Complete candidate profile: Curriculum Vitae (CV), current certification by the American Board of Surgery, Basic Life Support    (BLS), ATLS and ACLS mandatory for all specialties for each proposed candidate.

4.            The Contractor shall demonstrate:

a)            Competence in inpatient and outpatient surgical care.

b)            Competence in surgery procedures

c)            Successful application for credentialing by the medical staff of GIMC

d)            Maintenance of professional certification through participation in appropriate programs for continuing medical and pharmaceutical education.

5.            Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile

6.            Review of references (at least 2)

7.            Submittal of Option CLIN

8.            Current Copy of Certificate of Insurance for each candidate

9.            Price    

Quotes will be due on or by 06/28/2022.  The acquisition is IEE/Buy Indian Set-Aside.
