506-334 – Construct Private In-Patient Rooms and Primary Clinics – Ann Arbor, MI

Amendment 0001: Provide site visit attendee sign-in sheet and clarify the due date for technical questions. Questions are due July 10, 2024 at 1:00pm ET and not July 9 as the site visit agenda incorrectly stated. There are no further changes to the solicitation terms and conditions. See attached amendment which must be acknolwedged on the bid submission.

**PLEASE NOTE: Amendment 0001 does not have an SF30 to sign and return. Please acknowledge the amendment on the Bid Standard Form 1442 in Box 19. All other amendments with an SF30 must be signed and returned with the bid along with acknowledging all amendments on the Bid Standard Form 1442 in Box 19.**

Amendment 0002: 

The purpose of Amendment 0002 is to provide the following:

1. See BLOCK 11 of this amendment for the revised due date and time.
2. Responses to technical questions. See attachment titled Technical Questions - Ann Arbor - 506-334 – FINAL - 7-16-24.
3. Supplemental documentation concerning responses to technical questions (Revised drawings).
4. To provide the bid opening call-in line:

An in person bid opening will not be facilitated. Offerors may participate in the bid opening via teleconference line. Offerors may participate in the bid opening by calling 1-872-701-0185. When prompted for the access code, please enter 966781541 followed by the # key. This conference line will be available on Wednesday July 24, 2024 at 1:30 PM (ET). Please be advised that bid submissions are still due at 1:00 PM (ET), on this date.

There are no further changes to the solicitation terms and conditions. See attached amendment which must be acknolwedged on the bid submission.

The purpose of Amendment 0003 is to provide the following:

1. See BLOCK 11 of this amendment for the revised due date and time.
2. Advise vendors additional questions and revised drawings will be provided in a separate amendment.
3. Advise vendors there is NO change to the call-in line information provided in amendment 0002. The conference line will be available on Wednesday July 31, 2024 at 1:30PM (ET). Please be advised that bid submissions are still due at 1:00PM (ET) on this date.

The purpose of Amendment 0004 is to provide the following:

1. Responses to additional technical questions (questions 80-89). See attachment titled Round 2 Technical Questions - Ann Arbor - 506-334 – FINAL - 7-22-24.
2. Supplemental documentation concerning responses to additional technical questions (Amended Drawings - 7-22-24 and Plumbing Drawings Write-Up). The revised drawings are incorporated into the Amended Drawings dated 7-22-24. All documents referring to “addendum” are revised to “amendment”.

The purpose of Amendment 0005 is to provide the following:

1. Updated Wage Determination for Washtenaw County. Attachment 14 - Wage Determination - Washtenaw County - 4-5-24 is updated to Attachment 14 - Wage Determination Rev 6 - Washtenaw County – 7-26-24.
