Description

This solicitation notice is a request for a Non-Personal Service Contract for a Contractor to provide registered nurse services for Northern Navajo Medical Center, (NNMC) - Navajo Area Indian Health Service (IHS).  Services shall be performed in accordance with the attached Performance Work Statement (PWS).

These Request for Quote (RFQ) is Unrestricted for a Non-Personal Service Contract per FAR 37.401(a) for RN services to be performed at the three locations.

The Period of Performance will be issued 6 months each 1) Base Period 2) Option 1 Period

Base Period: Date of Award through 6 months

1st Option Period: Extend additional 6 months

The assignment is through the stated period of performance or until the position is filled with a permanent hire.  In such instances, the nurse will be released from contract assignment.

IDENTIFY RN DEPARTMENT WHEN SUBMITTING PROFILES

Location - NNMC:

RN – MCH-OB/GYN Out Patient – 3 Positions

 – Attach profile with current: BLS certificate

Attached RN profile with certifications – Not separated from profile. This is submitted to the department supervisor as 1 complete profile package.

** It is the responsibility of the Offeror to submit only qualified, applicable, and available personnel. Any personnel submitted by multiple Offerors will not be forwarded for evaluation and further consideration. **

WORK SHIFT:

The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The Supervisor at IHS shall determine specific tour of duty.

The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated.

HOUSING:

No government housing available for NNMC. The nearby towns are Farmington, NM , 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM. There are hotels, restaurants, mall, etc. in the two towns

REQUIREMENTS:

NOTE: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. In addition, any profiles received before or after the RFQ dates WILL NOT be accepted.

1. Quotation in the line items above. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The Attach Wage Determination Service Contract Act applies.

2. Past Performance: Provide experience of comparable scope and complexity in providing services within the past three (3) years of operation.

3. Qualifications: Provide complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information- phone number/email address, Professional Nursing Skills and all relative documents for proposed candidate. Refer to PWS Section 5.0 & 7.0

4. Availability of candidate(s).

5. The Contractor will start until the Purchase Order is signed by Contracting Officer

6. Certificate of Liability Insurance (COI) for each candidate also be considered, should one or more quotes represent the best value

7. U.S. Citizenship is required

8. Return attached Certification Statement and indicate DEPARTMENT for each candidate.

9. Return Open Market Pricing on separate sheet

10. Identify on the email subject line the solicitation Notice ID name

QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant factors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. All candidates are required to have an official United States social security number.
