Description

The Centers for Disease Control and Prevention (CDC) requires one (1) new firm fixed price purchase order for labor, materials, equipment and perform all work necessary to collect Adult Onset Respiratory Recurrent Papillomatosis (ARRP) data from a variety of otolaryngology clinical sites, oversee collection of data, and format and submit data  to CDC.  This purchase order is anticipated to be for a base period of eleven (12) months, with one option period of twelve (12) months. 

All quotations/responses received within fifteen (15) days of the issuance of this notice will be considered by the government.
