The information gathered from this request for information (RFI) is for planning purposes only. The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this Sources Sought Notice. The Miami VA Health System is seeking business concerns capable of providing the Maimi VA Medical Center Facility address listed below: Miami VA Medical Center 1201 NW 16th Street, Miami, FL 33125 The contractor shall furnish Professional Services to provide. all labor, equipment, tools, material, supervision and other items and services necessary to perform the 10- Year Sanitary System Evaluation Survey to perform in accordance with DERM requirements at the Dept. of Veterans Affairs Medical Center, Miami, Florida. Contractor shall coordinate and manage the contractors field investigation, sewer site mapping, smoke, and flow measurement testing services and will coordinate and manage any repairs and or video inspection of sewer system when required. Contractor will prepare all certifications, documentations and contractors report with engineers signed and seal certificate and final SSES report. Contractor will complete an on-site visual field investigation and evaluation of existing sewer system and the 4 pumping stations. Contractor will perform 4 smoke tests of the sewer system per DERM s requirement and up to 18 manholes. Contractor will provide a report with photographs for the property s SSES per DERM s requirement. The contractor will provide a final report. Firms with interest and capability to include, Federal Schedule Contract Holders, Small Business, Veteran-Owned Small Business, Service-Disabled Veteran-Owned Small Business, HUB Zone Small Business, Small Disadvantaged Business, and Women-Owned Small Business Concerns and all other business types and sizes may respond by submitting a qualifications letter stating the following information: Name Capability Statement (to include the Counties/Locations of interest) DUNS # and SAM UEI Info-Must be registered at SAM.GOV site.
