SOLICITATION

The contractor shall provide yoga therapy to patient populations of Veterans at the Michael E. DeBakey VA Medical Center (MEDVAMC). Contractor’s personnel shall perform services via video teleconference (VTC). Contractor shall provide all access to the necessary resources to accomplish two-way, interactive, virtual platforms to offer online classes and individual sessions of live yoga therapy.

The solicitation will be issued as a Small Business (SB) set-aside under NAICS code 611699, with a small business size standard of $16.5 Million. All interested parties must be listed in the System for Award Management (SAM) at https://www.sam.gov, at the date and time of their quote submission, and date of award.  Award will NOT be delayed for registration.  

SPECIAL INSTRUCTIONS

1. All quotes must be submitted directly to the Contracting Officer identified and listed in block 7a of the Standard Form (SF) 1449, by email to the following address: surella.tracy@va.gov. Quotes must be received no later than the date/time indicated in block 8 of the SF 1449. Any proposal received after the date and time referenced in block 8 may be considered late and may not be accepted.

2. All emails must reference “RFQ 36C25624Q0701, “Yoga Therapy” in the subject line of the email. Failure to do so may result in mishandling or inadvertent deletion of emails.

3. QUESTIONS: No more questions accepted.

4. SUBMISSION REQUIREMENTS: All quotes must include the following, as a minimum. Failure to include any of the following with a quote may result in that quote being determined non-responsive. Any quote determined non-responsive will NOT be evaluated for award.

a. Contractor shall submit proof of updated certifications with quote as outlined in Attachment 1 - Statement of Work, Section 3.2.  

b. Contractor’s business name (as registered in SAM)

c. Contractor’s address (as registered in SAM)

d. Contractor’s Unique Entity ID number (as registered in SAM)

e. Point of Contact (POC) Name

f. POC phone number

g. POC email address

h. Completed Price Schedule (Section B.1)

i. Completed Contract Administration Data (Section B.2)

j. Completed solicitation provision 52.204-24 (Section E.1)

k. Completed solicitation provision 52.209-7 (Section E.2)

l. Completed solicitation provision 52.212-3 (Section E.7). Complete and return only paragraph (b)(2) of this provision if your annual representations and certifications in SAM, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard(s) applicable to the NAICS code(s) referenced for this solicitation), at the time this offer is submitted.
