Description

This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice.  A quote is being requested and a written solicitation will not be issued.  Requirement number OBRHI-393-2024-0017 is issued as a Request for Quotation.  The Centers for Medicare and Medicaid Services (CMS) intends to award a fixed price purchase order to a contractor using Simplified Acquisition Procedures for the purchase of 40 Mural user Licenses in accordance with the attached Statement of Work dated July 8, 2024.  The period of performance will be for 12 months. The purchase order resulting from this RFQ will be awarded to the responsible Offeror whose quote, conforming to the solicitation, is determined to provide the “Lowest Price that is Technically Acceptable to the Government”.  CMS reserves the right to award based upon the initial quote; therefore the initial quote should contain the Offeror’s best terms. CMS also reserves the right to conduct exchanges with fewer than all Offerors.  The NAICS Code is 513210 – Software Publishers. Your response shall be no more than 5 pages and shall include the following: 1) Contractor’s plan to perform the Statement of Work; 2) and the firm fixed price. All responses will be evaluated using the following Evaluation Criteria: 1) Technical Understanding 2) Personnel Qualifications, 3) Management Approach and 4) Price (Base Year).   Your response may be sent electronically in MS Word format or it can be submitted via mail in hard copy.

Your quote should include the following additional information:

1.  Company Name, Address

2.  Contact Person, name, Telephone Number, E-Mail Address

3.  Business Size Classification, i.e. Small Disadvantaged, 8(a), Woman Owned

4.  UEI Number

5.  Tax ID NO.

Note: You must be registered and the registration current in the SAM or System for Award Management in order to receive a federal government award.

Your quote must be received by August 27, 2024 no later than 12:00pm.
