Description/Scope of Work

This is a combined synopsis/solicitation for commercial products and installation prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6 as supplemented with additional information included in this notice.  This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued.

This solicitation is a request for quotes (RFQ).  The solicitation document and incorporated provisions and clauses are those in effect through FAC 2023-02.

The associated North American Industrial Classification System (NAICS) code for this procurement is 811210 with a business size standard of $34.0.  The product or service code is J049.  The Government will award a firm-fixed priced (FFP) purchase order resulting from this combined synopsis/solicitation to the responsible offeror whose offer, conforming to the synopsis/solicitation, will be most advantageous to the Government (as determined by the solicitation evaluation criteria).

Santa Fe Indian Health Center (SFIHC), 1700 Cerrillos Road, Santa Fe, New Mexico, is seeking to purchase an Annual Preventive Maintenance and Service on the Zeiss HFA-3 Model 860 (Serial Number 860-22100) Visual Field Analyzer with the following specifications:

The contractor shall provide an Annual Preventive Maintenance and Service on the Zeiss HFA-3 Model 860 (Serial Number 860-22100) Visual Field Analyzer.

• Unless otherwise noted in this document, included are:
  • Two (2) Operator Manuals
  • Two (2) Service Manuals
• One Year Warranty on all products except Lasers that have a Two Year Warranty.
• Technical Support via remote telephone conversation as it relates to appropriate network ports and settings.
