The purpose of this amendment is to address industry generated questions and answers, as well as to revise the Statement of Work, dated 02 November 2023, revise the Attachment 1-Pricing Worksheet, and add Attachment 2-Quality Control Numbers. The request for quote deadline is also extended to NL T 09 November 2023 at 3:00pm CST.

The Medical Readiness Contracting Office – West (MRCO-W) Fort Sam Houston TX is issuing a Request for Quote (RFQ) for two leased Chemistry and Immunoassay Integrated Analyzers and Reagent Testing Supplies under a Cost per Reportable Result (CPRR) agreement which includes equipment, reagents, tools, materials, training, and water purification system, Uninterruptable Power Supply (UPS) and services necessary to meet the Chemistry and Immunochemistry testing and analysis requirements for Bayne Jones Army Community Hospital (BJACH), at Fort Johnson, Louisiana.   

This acquisition is conducted as Unrestricted Full and Open Competitive.  The North American Industrial Classification System (NAICS) Code that best fits this requirement is– 325413, In-Vitro Diagnostic Substance Manufacturing.  The NAICS code selected best fits PSC 6550 – In Vitro Diagnostic Substances, Reagents, Test Kits and Sets.  Small Business Size Standard: 1,250 number of employees. All eligible businesses may submit an offer, which will be considered.  It is required that all business contractors' doing business with the Government be registered with the System for Award Management (SAM) at website https://sam.gov/.  The government anticipates awarding a Cost per Reportable Result (CPRR) Firm-Fixed Price (FFP) contract with a base period of twelve months.   

Period of Performance duration will be from 01 April 2024 to 31 March 2029 plus four option years. 
Base Year: 01 April 2024 thru 31 March 2025 
Option Year 1: 01 April 2025 thru 31 March 2026 
Option Year 2: 01 April 2026 thru 31 March 2027 
Option Year 3: 01 April 2027 thru 31 March 2028 
Option Year 4: 01 April 2028 thru 31 March 2029 

Independent consultation with Government Officials concerning the project requirements is prohibited since evaluation of quotations will be based on requirements stated in the Request for Quotation.  Industry Generated Questions regarding the design, coordination, or interpretation of RFQ requirements during the solicitation phase shall be in writing and directed to the Contract Specialist, Ms. Tammi Lambert at tammi.r.lambert.civ@health.mil, with a courtesy copy to J. Chris Finkenstadt at 
john.c.finkenstadt2.civ@health.mil no later than 27 October 2023 at 2:00 P.M. (CST).  Questions submitted after the question due date will not be accepted.   

Complete quotation is due NLT 2:00PM CST on 6 November 2023.  Deliver to U.S. Army Health Contracting Activity/ Medical Readiness Contracting Office – West electronically, ATTN:  Ms. Tammi Lambert at tammi.r.lambert.civ@health.mil and J. Chris Finkenstadt at john.c.finkenstadt2.civ@health.mil . It is each contractor’s responsibility to ensure their quote and all data is complete and IAW the RFQ. The government assumes no responsibility for late or incomplete responses to the RFQ. Late responses will be processed IAW FAR 52.212-1(f).
