Description

The Network Contracting Office (NCO) 12, Great Lakes Acquisition Center (GLAC) is seeking one contractor to furnish Surgical Laser Rental and Technician services at the William S. Middleton Memorial Veterans Hospital, 2500 Overlook Terrace, Madison, Wisconsin (hereinafter called VA) under the authority of FAR 13.5, Simplified Procedures for Certain Commercial items. The resultant contract will consist of a five-year ordering period contract with an anticipated award date of February 1, 2023. All interested parties who believe they can satisfy the solicitation requirements are invited to provide a response to this Request For Information (RFI). The contractor shall provide all CO2 Omni-guide, CO2 Direct line of Site, Green Light, and KTP, Aura, Aura XP, Deka, TruBlue laser equipment, accessories, fibers, and qualified technician services to be available for laser equipment malfunction. Equipment and services shall be provided by the contractor on a case-by-case basis in accordance with the contract requirements. All laser equipment, including the laser, accessories, and fibers shall be in optimum operating condition. The Madison VA physician shall perform the procedures in the Operating Room (OR) at the Madison VA Hospital. The Madison VA Hospital OR Business Manager or designee will notify the contractor by telephone and Madison VA Healthcare Technology Management of a treatment requiring laser services a minimum of forty-eight (48) hours in advance of the surgery to be performed. The OR Business Manager or authorized designee are the only authorized individuals to order Laser Services. Laser services will only be requested during business normal hours of operation, Monday through Friday from 7:30 a.m. to 5:30 p.m. Laser services will not be requested after normal business hours, weekends or holidays on this contract or paid through this contract.
