MEDICAL PHYSICIST INSPECTIONS

SOLICITATION

The Central Arkansas Veterans Healthcare System (CAVHS) has a requirement for Medical Physicist Inspections at its main hospital facilities located at 4300 W. 7th St, Little Rock AR,  72205 and 2200 Fort Roots Drive, North Little Rock, AR 72114; and VA operated Community Based Outpatient Clinics (CBOCs) located at 1520 East Dave Ward Dr., Conway,  AR 72032;  3106 West 2nd Ct. Russellville, AR 72801;  1120 South Main Street, Searcy, AR 72143;  and 177 Sawtooth Oaks St. Hot Springs, AR 71901.

This is a total Service-Disabled, Veteran-Owned, Small Business set-aside under NAICS code 541690.  All interested parties MUST be listed in the U.S. Small Business Administration’s Veteran Small Business Certification (VetCert) database at https://veterans.certify.sba.gov/ and the System for Award Management (SAM) at https://www.sam.gov as of the date of their quote and date of award in order to be eligible.  Neither the quote due date nor award of the contract will be delayed for registration.

Interested parties must submit all questions concerning the solicitation, in writing, via email to james.hunt2@va.gov, to be received no later than 4:00 PM, Monday, June 10, 2024.  Questions received after this date/time may not be addressed prior to the quote due date.

As indicated on the cover page, all quotes must include the following to be considered for award:

- Contractor’s business name (as registered in SAM)

- Contractor’s address (as registered in SAM)

- Contractor’s Unique Entity ID number (as registered in SAM)

- Point of Contact (POC) Name

- POC phone number

- POC email address

- Completed Price Schedule (Solicitation Section B.2)

- Completed Offeror information in Solicitation Section C.6 (VAAR 852.219-75)

- Completed Offeror Representations and Certifications (Solicitation Section E.4). Complete and return only paragraph (b)(2) of this provision if your annual representations and certifications in SAM, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard(s) applicable to the NAICS code(s) referenced for this solicitation), at the time this offer is submitted.
