Description
This requirement is being issued as 100% Small Business Set-Aside. (Ref: FAR 19.5)
This requirement will be competed and awarded under Federal Acquisition Regulation (FAR) Part 13, Simplified Acquisition Procedures.
The North American Industry Classification System (NAICS) code for this service is 624190. The small business size standard is $14 Million average annual revenue for the previous three years.
The Governmnet intends to select one Contractor from the subject solicitation.
Description of the Service Requested: Short-term diagnostic assessment, intervention, counseling and referral services for various issues affecting employee mental and emotional well-being, which may adversely impact the employee's job performance no limited to work-related difficulties such as alcohol and other substance abuse, stress, grief, depression, anxiety, martial and family problems, eating disorders, financial/credit problems, legal concerns management consultation, organizational development services, training and education awareness; traumatic/critical incident support services, as defined in the PWS. Long term treatment and rehabilitation is not within the scope of the contract. The goal of the EAP is to enhance employee and workplace effectiveness through assisting employees with personal problems and productivity issues that may adversely impact their work performance.
The Period of Performance is for one year starting on or about 1 April 2023. The resulting purchase order will be a firm-fixed price with a base year period and four one-year option periods.
By submittting a quote in response to the RFQ, Contractors validate their quatation will be valid for 90 days from the RFQ due date.

All interested parties submitting quotes, that do not have an active account in System for Award Management database at http://www.sam.gov website will NOT be eligible for award.
