Description

The Indian Health Service, Gallup Service Unit, Tohatchi Health Center located 07 Chooshgai Drive, Tohatchi, NM has an upcoming requirement for non-personal service contract for physician services. 

Period of Performance:

Base Period: August 1, 2022 – December 31, 2022

1st Option Period: January 1, 2023 – March 31, 2023

2nd Option Period: April 1, 2023 – June 30, 2023

3rd Option Period: July 1, 2023 – September 30, 2023

The North American Industry Classification System or NAICS code is 621111, Office of Physicians with a small business size standard of $14 million.  

The Request for Quotations No. 75H71022Q00053 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by June 24, 2022.  Evaluation factors may include experience, licensure, eligible for or possessing Board certification, documentation of completion of a family medicine or internal medicine residency program, possession of a valid driver’s license, and certifications.  Quotes will be due on or by July 6, 2022.  The acquisition is a total Indian-Owned Economic Enterprise (IEE) set-aside.

In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
