Description

The Indian Health Service, Gallup Service Unit, located in Gallup, New Mexico, 87301 has an upcoming requirement for three (3) Full-Time Non-Personal Healthcare Services for Orthopedist Physician Services in the Orthopedics Department.

The period of performance will be as follows:

Base Period: 01/01/2023 – 03/31/2023

1st Option Period: 04/01/2023 – 06/30/2023

2nd Option Period: 07/01/2023 – 09/30/2023

3rd Option Period: 10/01/2023 – 12/31/2023

The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $12.0 M. 

The Request for Quotations No. 75H71023Q00011 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 12/05/2022.  Evaluation factors may include:

Required capabilities and performance characteristics:

1. Provider shall provide professional medical services in the Orthopedics Department. Required to take calls and be responsive to Trauma Orthopedics patients that enter the Emergency Room.

2. Contractor’s physician experience of the past thirty-six (36) months and a minimum of twelve (12) in their specialty unless otherwise approved by the Contracting Officer.

3. Contract Requirements and must show the following:

  -Board Certified/Board Eligible in Orthopedics

  -Competency in Orthopedics

  -Confidentiality of patient care and records

  -Credentialing by Medical Staff

  -GIMC Contractor’s Licenses/Certifications, MANDATORY:

    a) Current Basic Life Support (BLS)

4. Residency in Orthopedics

5. Two (2) References of the Contractor

6. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile

7. Submittal of Option CLIN

8. Current Copy of Certificate of Insurance for each candidate

9. Price

10. Able to start as soon as possible

Quotes will be due on or by 12/09/2022.  The acquisition is Unrestricted.
