Description

AMENDMENT 0001: Amendment incorporates a corrected Performance Work Statement. See attachment titled, "Attachment 1 Performance Work Statement (Amendment 0001)." No other changes.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued.

This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-04.

The associated North American Industrial Classification System (NAICS) code for this procurement is 621111 with a small business size standard of $16 million.

The contractor shall provide Occupational Health Medical Examination Services in the Waterloo area, as described in Attachment 1, Performance Work Statement (PWS). Contract line item numbers (CLINS) and quantities are as follows:

CLIN 0001: Physical/Dermatological Exams, Quantity/Unit of Issue: 29 Job

CLIN 0002: Vision Screenings, Quantity/Unit of Issue: 29 Job

CLIN 0003: Complete Lab Draw, Quantity/Unit of Issue: 29 Job

CLIN 0004: Lead Blood Level, Quantity/Unit of Issue: 29 Job

CLIN 0005: Chromium Blood Level, Quantity/Unit of Issue: 29 Job

CLIN 0006: Cadmium Blood Level, Quantity/Unit of Issue: 20 Job

Occupational Health Medical Surveillance Examination Services shall be FFP.

The period of performance is date of award through 60 says after award,

Place of Performance is within a 40-mile radius of:

2245 West Big Rock Rd.

Waterloo, IA 50036

Award will be made to the offeror whose quotation meets all requirements of the PWS and solicitation, and offers the lowest total price to the Government.

See Attachment 2 for FAR and DFARS provisions and clauses which are hereby incorporated into this solicitation. The clauses will be incorporated into the resultant purchase order.

Offerors must complete annual representations and certifications online in the System for Award Management in accordance with FAR 52.212-3, “Offeror Representations and Certifications–Commercial Products and Commercial Services.” If paragraph (b)(2) of the provision is applicable, a written submission is required.

All offerors must submit the following:

  • Name and address of proposed examination locations
  • Completed Contact Information and Pricing Table (Attachment 3)

Questions are due by 10:00 a.m. local (central) time on 25 Aug 2023. Questions shall be submitted via email to kelsey.l.letcher.civ@army.mil. and vicky.l.williams10.civ@army.mil. Questions will not be accepted by phone. Questions and answers will be consolidated into a single Q&A document and posted to Contract Opportunities on www.sam.gov on a non-attribution basis. Offerors shall monitor the website for the most current Q&A documents and amendments to the solicitation.

Quotes are due by no later than 10:00 a.m. central (local) on 31 Aug 2023.  Quotes shall be submitted via email to Kelsey Letcher, kelsey.l.letcher.civ@army.mil.  The contractor is responsible for ensuring quotes are submitted by the due date and time indicated. Late submissions will be treated in accordance with the solicitation provision at FAR 52.212-1(f).
