Description

To be considered for this requirement you must complete the SF 1449; no other format will be considered.

1. Respond to blocks 17a and 30 a-c on page 1.

2. Complete the vendor information on page 3

3. Provide your price(s) on the CLIN on page 4

Defense Health Agency intends to award based on brand name for the required generator.  No other brand can satisfy agency requirements. The estimated time of delivery will be 45 days after receipt of order. This purchase will be made utilizing Simplified Acquisition Procedures in accordance with: Federal Regulation (FAR) Part 13.106-1(b) for purchases not exceeding the simplified acquisition threshold of $250,000.
