SCOPE OF WORK: 

The purpose of this project is to provide construction services for the renovation and modification of the Desert Visions Youth Wellness Center in Sacaton, AZ.  

The primary goal of this work is to provide for the following:

  1. Landscape and provide irrigation of the grounds of Desert Visions
  2. Replace concrete panels and paint the court for basketball and pickelball
  3. Provide concrete sidewalk in the courtyard between dormitories
  4. Seal west parking lot
  5. Resurface east parking lot
  6. Provide new roofs for all buildings
  7. Provide new epoxy floor in the walk-in freezer
  8. Replace flooring in the serving & dining area with tile
  9. Resurface the pool area, provide anchor point for basketball hoop & replace pool system
  10. Provide Cured-In-Place Pipe interior to and exterior of building 100
  11. Provide new plumbing fixtures

LOCATION OF PROJECT: The Project is to be performed at the following site: Sacaton, AZ.

TERO: TERO fees do apply.

PERIOD OF PERFORMANCE: The contract Period of Performance is 180 calendar days from date of notice of proceed.

CONSTRUCTION MAGNITUDE: The magnitude of this construction project is anticipated to be between $500,000 and $1,000,000, in accordance with FAR 36.204.

SITE VISIT: Details regarding the Site Visit will be included in the solicitation.

CONTRACT TYPE: The Indian Health Service intends to award a Firm Fixed Price (FFP) contract in support of this requirement. This solicitation will be procured as a Request for Proposal (RFP) in accordance with FAR Part 36 procedures with Lowest Price Technically Acceptable evaluation criteria.
