Last Updated: 12/20/2023 Oklahoma City Veterans Affairs Health Care System (OKC VA HCS) Statement of Work (SOW) Soil Testing Behind Building 21 I. GENERAL: This project is to test soil behind building 21 at the OKC VA HCS at Oklahoma City, Oklahoma. The contractor is fully responsible for verifying and validating all information contained within this statement of work. Contractor must fully evaluate the condition of the existing structures and verify the current condition of the project site. All measurements/quantities provided in this document are approximate and are to be used for reference only. The contractor will be responsible for taking on-site measurements and verifying all quantities and distances. All materials/fasteners/etc. must be new and provided by the contractor unless otherwise specified. The contractor shall abide by all Safety, Security and Infection Control Policies and Procedures of the Oklahoma City VA HCS. VA facilities are tobacco free. No smoking is permitted on VA property. The contractor is responsible for their subcontractor s compliance with this policy. All work must conform to, and comply with, the latest edition/version of ALL applicable VA, federal, state, and local laws, rules, regulations, codes, and standards. II. JUSTIFICATION: Geotechnical company was performing testing and while drilling they smelled diesel fuel and possibly hit concrete as the drilling could not go further down as expected. III. STATEMENT OF WORK: The contractor shall provide supervision, manpower, and materials perform soils testing of the area designated by the COR. Area is behind building 21. Contractor shall perform soil sampling at 4 locations within area. Contractor shall collect 1 soil and 1 groundwater sample per bore hole/temp well Perform decontamination and plugging of soil borings when complete Perform waste profile for drum disposal at Waste Connection OKC Landfill Contractor shall utilize OCC-Licensed consultant as required by the Oklahoma Corporation Commission (OCC) Contractor shall provide licensed driller and labor to perform installation of temp wells and plugging activities as required by the OCC Contractor shall record and sample under appropriate chain-of-custody procedures per OCC regulations Regulatory coordination with the OCC and local municipalities as required by the OCC Provide Analytical Report and Data to submittal through the OCC Portal (as required by the OCC) Contractor shall dispose any trash or debris they generate during the duration of the performance period and completion of project Clean up area after project completion and contact COR for final inspection Performance Period: 30 days IV. PHOTO REFERENCES: N/A V. WORK SCHEDULE Normal working hours are from 7:00 AM to 3:30 PM (local time) Monday through Friday except for weekends and established Federal Holidays.
